The relationship with a patient begins before the actual visit. How can the care team efficiently gather information about the patient, their circumstances, and the reason for the visit? How can we ensure that we gather the pertinent data at the right time so that it will be available to the care team in the right place and at the time of visit?
Care team/patient workflow begins as the patient arrives and is checked in for the visit and ends when the patient checks out and departs our facility. At every point during the visit data are collected and organized so that it can be used to help resolve the presenting problem. How can we avoid the train wrecks that happen every day when data are not available when needed in the process? How can we ensure that the patient leaves with a thorough understanding of their care plan?
Many of the factors behind a patient’s health are closely linked to social services that have not traditionally been a part of the care plan. How can we incorporate social services like housing, food, transportation and other social services into our care model? How can we order these services for a patient and receive results of the ordered service in order to document the impact on a patient’s return to health?
We need to be able to meet our patients where they are rather than expecting our patients to come to where we are. Engaging our patients as a full partner in care requires that we think beyond our clinic walls. How can we provide the highest clinically appropriate level of care possible in settings that are convenient for the patient?
