California Practice Network

Health AI Partnership (HAIP) and CCI in collaboration with the California Health Care Foundation (CHCF) are thrilled to launch the California Practice Network!

The Practice Network is an initiative designed to help organizations accelerate responsible AI adoption and bridge the digital divide in safety-net healthcare. Health Center Controlled Networks (HCCNs), Regional Associations of California (RACs), and state and regional consortia in California interested in developing organizational capabilities for AI adoption using HAIP best practices are invited to apply. Participating organizations will receive customized support through no-cost expert advice, peer learning networks, and curated educational resources to enhance the technical assistance they deliver to member health centers or clinics implementing AI.

Idea Labs for Immigrant Health Access

Immigrant communities across California are losing coverage and losing trust at the same time. State and federal changes are narrowing who qualifies for Medi-Cal, and enforcement activity is keeping people away from clinics. At most risk are adults with unsatisfactory immigration status, who gained full-scope Medi-Cal only recently and now face new limits on it.

CCI is committed to supporting improved access to care for immigrant community members. As a first step, we plan to bring cross-sector partners together in up to five California regions for a full day of structured, hands-on problem solving. We call these Idea Labs. Each one is organized around a single question: How might we build trusted pathways to care that help immigrant communities access and stay engaged in health care? Participants validate the main barriers in their region, develop shared strategies, and begin building something that outlasts the current crisis.

We’re using this brief Expression of Interest to learn where there is appetite, readiness, and the greatest opportunity for impact. Please share how a convening could be of value in your region, which barriers need the most attention, and where you see momentum. We’ll follow up with conversations, and what we learn will inform where and how the work might unfold.

Advancing Health Equity in Medi-Cal Managed Care

As managed care plans (MCPs) face increasing expectations to advance health equity, there is a growing need to understand which approaches are producing meaningful results and how those strategies can be adapted across California.

CCI and the California Health Care Foundation (CHCF) are seeking submissions from Medi-Cal managed care plans (MCP) that have successfully reduced health disparities and improved outcomes for Medi-Cal members. Through this Advancing Health Equity in Medi-Cal Managed Care initiative, CCI and CHCF will develop case studies and implementation resources that help health plans learn from and replicate effective interventions.

We are seeking interventions that have improved patient access, experiences, or outcomes for populations experiencing racial, ethnic, or linguistic disparities, or that have closed an identified disparity relative to a referent population. Relevant practice areas may include clinical quality outcomes, preventive care, behavioral health, maternal health, language access, culturally responsive care, appointment access, and member engagement or navigation support.

All submitting organizations will receive individualized assessment and recommendations from a panel of health equity, quality improvement, and managed care leaders. Selected participants will receive a professionally developed case study highlighting their intervention, outcomes, implementation process, and lessons learned.
