Learning Session #1: Learning, Reflecting, and Applying
This first in-person learning session was held on October 19, 2017 and focused on effective team based care, creating driver diagrams, and sharing project progress.
Serve the People (STP), an FQHC located in California, hosted a site visit where teams learned about STP's team-based care model, group visits and nurse co-visits, and more.
Teams from all the Transformation Accelerator health centers visited Community Health Center, Inc. (CHC, Inc.) in Connecticut. Teams learned about CHC, Inc.'s approach to team-based care, using data for clinical decision-making, the role of leadership, quality improvement and innovation in their organization.
