Final Grant Report Due 12/31 As part of the requirements for the Kaiser Permanente Transformation Accelerator Mid-Atlantic Region (KPTA) program, each team must complete a final report and submit online by December 31, 2018.

The report consists of three components and must be submitted online here:

  1. Final narrative report: please answer the questions thoroughly, including adding November data. Your customized narrative template was emailed to your team lead.
  2. Final expenditures report (download excel) – as indicated in the grant award letters, to receive the final installment of $10,000, your organization must demonstrate at least $10k spending on technical assistance. Please reflect this spending in your expenditures report. TA spending can include the cost of any training, coaching, travel to site visits, registration fees to attend in-person/virtual session, etc. The remaining $20k of the grant can be allocated for personnel and other costs.
  3. Post-program assessment (download excel): this is the same assessment that you completed for the baseline and should be completed in a similar manner, rating your organization where it stands as of December 2018.
