Learn about the six steps of providing planned care in these two virtual sessions on May 1st and May 22nd.

These sessions are taking place of the in-person meeting that was scheduled to be held in March, but had to be cancelled due to weather.

Dr. Carolyn Shepherd, formerly from Clinica Family Health Services, provides an overview of what planned care entails and why it matters. She then does a deep dive into the six steps of providing planned care. Dr. Shepherd is joined by Tierney Giannotti and Mary Blankson from Community Health Center, Inc. to give examples of how the different steps are implemented at their organization. Family and Medical Counseling Service and Greater Baden Medical Services, as well as other teams from the Kaiser Permanente Transformation Accelerator Mid-Atlantic Region program will share their experiences with providing planned care.
