The Kaiser Permanente Transformation Accelerator program hosted an optional site-visit to Serve the People, Inc., an FQHC in California on December 13, 2017.

Serve the People is an FQHC that started in 2011 and has earned level 3 PCMH recognition. In their brief time, they’ve built the infrastructure, team and financial solvency to provide comprehensive primary care services to multiple age groups and product lines. They also have integrated behavioral health, dental, and vision.

They hosted a site visit for two Transformation Accelerator teams, which included presentations, small discussions, open Q/A, videos, and a clinic tour addressing topics such as team based care with a focus on roles and workflows, expanding services through community partnerships, and using alternative visits like shared medical appointments, mobile clinics, and telephone visits to improve access.

Different levels of STP staff, including the ED, CMO, COO- equivalent, medical assistants, QI specialists, patient navigators, BH Director, and their nurse, presented and answered questions throughout the day. In addition, videos depicting the clinic flow were shown to participants along with a tour that showed exams rooms, co-location and service integration, and a
view of the group visit space.
