The Kaiser Permanente Transformation Accelerator program hosted site visit to Community Health Center, Inc., an FQHC in Connecticut on January 17, 2018.

Community Health Center, Inc. (CHC, Inc.) serves as a partner and content expert for the Transformation Accelerator program and is an FQHC with 14 primary care hubs. The organization integrates preventative care, treatment, and chronic disease management into patient visits, and is nationally recognized for its commitment to implementing innovative care models and cutting-edge technology in their sites. The Weitzman Institute (WI), CHC’s research and development arm, studies the impact of primary care on the underserved, develops educational modules on care transformation, and provides training for health professionals and students.

The site visit included a tour of CHC’s Middletown site, where attendees were able to talk to front line staff and learn about CHC’s setup, patient flow, team space utilization, complex patient care, QI activities, and the use of data and dashboards. CHC’s then presented its model of care, including their team-based care structure and team roles. Attendees participated in small table discussions with CHC staff on topics such as HIV, UDS measures, hypertension and diabetes.
