Since 2015, CCI has brought our expertise in building vibrant learning communities, leveraging experts, and sparking innovative thinking among PHASE participants in Northern California.
We are proud to work with our partners at the Center for Community Health Evaluation and Kaiser Permanente Community Benefit in helping safety net organizations adapt PHASE and profoundly impact the suffering and costs associated with diabetes and chronic heart disease. The 14 organizations participating in PHASE community for the 2020-2021 grant cycle include three main cohorts: public hospitals and health systems; regional clinic consortia; and community health centers in the Sacramento and Central Valley regions.
Serves as the project office, coordinates technical assistance, and supports a vibrant learning community.
