Evaluation Corner

The Center for Community Health Evaluation (CCHE) has been involved in evaluating the impact of the PHASE program for over 10 years.

In addition to providing comprehensive quarterly data analysis of the PHASE measures, CCHE’s evaluation involves phone interviews with project leads and staff and in-person site visits to the public hospital systems and consortia grantees. Thanks for your engagement in evaluation activities!

Evaluation Timeline (2020-21)

The evaluation reporting and deliverable timeline for PHASE 2020-2021 is to the right.

Data reports are due to CCHE one month after the end of each quarter. For example, 2020 Q1 data should be submitted by April 30. You will report data on these 2020-2021 PHASE Measures. An updated template will be provided soon. Please submit your data to Carly Levitz. CCHE will return dashboards to your team about a month after each deadline for quarterly review and discussion.

This video describes highlights from the 2017-2019 PHASE grant cycle, including data highlights that showcase accomplishments across the organizations that participated. These are the metrics that all PHASE 2020-2021 teams will report on. There are some changes to the measures from what was required in the past iteration of PHASE (2017-2019). These measures more closely align with UDS and HEDIS definitions.

This newly released evaluation report shares the 2017-2019 results from Kaiser Permanente Northern California’s investment in PHASE. KP NCal provided three-year PHASE grants in 2017 to 18 health care safety net organizations across the Bay Area, Central Valley, Fresno, and Sacramento.
