A Clinic and a School District Team Up to Help Kids

Staff at the Santa Cruz Community Health Centers and the Live Oak School District in the community of Live Oak, California, knew they had a number of high risk, high need kids in common.

Both clinic and school district staff were frustrated that, despite their best efforts, many of these kids were struggling — in school and out. Clinic and school district staff realized that there was a lack of communication and coordination between healthcare providers, educators, and parents, and that this was hampering their ability to address the needs of the kids’ and their families.

So the Santa Cruz Community Health Centers (SCCHC) and the Live Oak School District decided to build and fortify lines of communication between health care providers, educators, and parents to more effectively identify and help the at-risk children in their care. They set out to do so with the support of the 2018 cycle of Seeding Innovation (also known as Innovation Lab—”iLab” for short.
