Conference National Summit on SDOH: This conference, held in San Diego, will focus on health equity. Session topics include building sustainable cross-sector partnerships to promote health equity, measuring health impacts, and addressing health and housing. There are multiple tracks to choose from.

Exploring the Impact of Integrated Medicaid Managed Care on Practice-Level Integration of Physical and Behavioral Health: This brief and related infographic describes how integrated financing influences the coordination of physical and behavioral health services. It provides recommendations for how physical-behavioral health integration can be advanced in areas of data-sharing and quality measures, payment and business practices, and clinical practice and service design.
Case Study: Addressing Patients’ Social Needs with an Online Social Service Referral Platform: Learn about Northeast Valley Health Corporation’s experience testing One Degree, an online social service referral program.
Case Study: Building a Collaboration with West County Health Centers to Address School Absenteeism: Learn about how West County worked to combine school absenteeism data with the health center’s clinical data to identify ways the health center and school district could collaborate to reduce school absenteeism and support families’ health and education goals.
