Virtual Community Health Worker Outreach – Community Health Center Network hired more staff to boost Care Neighborhood, an intensive case management program that connects high-risk, high-needs patients to local resources (like food and housing) and prevents unnecessary hospitalizations by focusing on addressing the social determinants of health.
