Due Date

Final Narrative & Financial Report due to CCISubmit the narrative and financial report here. CCI will release the final installment of the grant award upon receipt of the final narrative and financial report, and no later than June 30, 2020.

  • Updated Narrative Questions – For those teams that haven’t yet submitted the final report, we’ve simplified the narrative questions to focus on how your population health work is being impacted right now and looking toward the future. If you’ve already submitted the report, no need to re-submit.
  • Financial Report Flexible Allocations – Considering COVID-19, we’ve provided flexibility in how you allocate any remaining PHLN grant dollars to provide compassionate care and services to your patients. This means that if you didn’t fully spend your grant on direct PHLN work, you may allocate funding for any COVID-19 or other population work your organization worked on in the last several months.

IHI COVID-19 Weekly Series – This weekly series occurs every Friday from 7-8am PT. Learn more.
RCHC COVID-19 Podcast Series – Redwood Community Health Coalition has started focusing their podcasts on COVID-19 and sharing information that might be helpful for health centers, from telehealth to self-care. Listen here.
Virtual Community Health Worker Outreach – Community Health Center Network hired more staff to boost Care Neighborhood, an intensive case management program that connects high-risk, high-needs patients to local resources (like food and housing) and prevents unnecessary hospitalizations by focusing on addressing the social determinants of health. Learn more about their training, workflows and more related to this work.
Food Insecurity Resources – This is a guide to public, private, and nonprofit resources that address food insecurity, with a particular emphasis on California, for clinic clients who need food relief right now during the pandemic.
Solutions Testing for Food Insecurity in Los Angeles – Six clinics that are participating in CCI’s Moving Clinics Upstream are testing solutions for different demographics. Read about what’s being tested.
