Population Health Learning Network Informational Webinar

CCI hosts an informational webinar for safety net healthcare organizations interested in applying to the Population Health Learning Network.

Megan O’Brien, Program Manager at CCI, provides a program overview and information about how to apply.


California-based safety net healthcare organizations, that provide comprehensive primary care services to underserved populations, who are interested in participating in a learning network to further their population health management work.


  1. The PHLN is a two-year network where organizations will commit to attending three in-person convenings, participate in evaluation activities, and share and learn along with their peers, while also having the opportunity to select from a robust array of technical assistance and support depending on their needs in managing population health.
  2. Up to 30 organizations in California will be selected to participate in this two-year network focused on learning, sharing best practices, and taking action to advance population health.
  3. Applications are due online on Friday, February 9, 2018 at 5 pm PST.
