During this webinar, we highlighted two emerging bright spot topics we gathered from the quarterly report and Baseline Assessment submissions we received from PHLN teams.
Chapa-De Indian Health and Axis Community Health were asked to present their progress on diabetes group visits and behavioral health disease registries. Each team shared their lessons learned and advice from implementing or testing their respective projects. Webinar participants were also invited to engage in the discussion and share their thoughts and resources on the topics.
