We held the first in-person learning session for Transforming Cardiovascular Care in Our Communities (TC3) on November 5, 2019 in the City of Industry, CA.

We discussed application of best practices for blood pressure measurement and care management with Kaiser Permanente physician leaders and care team members, built leadership and influence skills to make improvement plans a reality by learning from experienced safety net leaders, and developed a problem statement, SMART aim, and driver diagram for the TC3 initiative.
