PHASE/TC3 – March 23, 2021 Final Virtual Session

This was the final virtual learning session for the PHASE and TC3 programs.

During this session, we had the opportunity to celebrate program accomplishments, spotlight the hard work of PHASE and TC3 teams, and reflect on how the last year challenged us to advance our thinking about the possibilities of caring for patients with or at risk for cardiovascular disease.

Leaders from Northern and Southern California Kaiser Permanente – who have been founders and champions of these programs over the years – shared their reflections on the evolution of the program, highlighting how far we’ve come and where they see the possibilities for the future as we strive to provide high-quality care for our patients in this ever-changing world.

PHASE & TC3 teams shared their accomplishments and pitched their vision for continuing and sustaining this important work. This took place in small groups with each team’s leadership present to offer their own reflections.
