This website is a support center for the use of Sustainable Models of Telehealth in the Safety Net participants. Program updates, report due dates, resources, newsletters and more will be posted to this website. This website is managed by Center for Care Innovations.

Program Goal

The goal of the is to expand access to and use of specialty telehealth services among low-income Californians and to develop sustainable telehealth programs. Project grantees agree to set a goal and work toward increasing their volume of telehealth (live video and store and forward) by 1,000-2,000 visits per year by the end of the two-year grant period.

Program Components

Report volume and usage data monthly

Participate in peer-learning webinars and in-person meetings

Develop a long-term sustainability plan for telehealth for your health center

Engage in external evaluation of the project outcomes related to access, satisfaction, and costs
