With the completion of the Expanding Access through Team Care program, the program evaluators wrote case studies on three participating health centers.
In 2014 Blue Shield of California Foundation, in partnership with the Center for Care Innovations, launched the Expanding Access Through Team Care program.This program provided funding and technical assistance to health centers to improve their care teams and implement solutions that would allow those teams to see more patients. With the program now complete, the Center for Community Health and Evaluation examined the program grantees and created three case studies of the effect of this initiative on the following three organizations:
