As a part of our Health Homes for Complex Patients program, CCI is sharing a series of tools related to complex care that were presented at a convening held jointly by CCI, the California Improvement Network and the California Healthcare Foundation.
The package of resources contains forms developed by the Alemeda Health System’s Hope Center, San Francisco Health Network and Care Oregon to manage their complex care team. This package includes:
- A sheet on the role of community health workers from Hope Center.
- A list of health coach tasks over time from SFHN.
- A weekly meeting prep list from SFHN.
- A case presentation form for team huddles from Care Oregon.
