On July 10th, grantees in the PHASE program had the opportunity to visit one of four different organizations who highlighted innovative population health management practices to improve care for patients.

After the site visits, participants enjoyed lunch and a debriefing session at the Redwood Community Health Coalition to synthesize what they learned from the site visits and to discuss what they could take back to their own clinic/system.
