Marin Community Clinics
In an effort to improve on quality measures, care teams at Marin Community Clinics have protected time once a month to plan care and outreach to patients. These population health management sessions facilitate consistent evidence based care via panel management and enlists the care team including RNs, patient navigators, front office, and medical records, as well as the provider and MA teamlets.
