Appendix: Connected Care Accelerator Measures
The purpose of the Connected Care Accelerator measures is to support teams in understanding their progress over the course of the program and to understand how the virtual care efforts are advancing across the participants. The Accelerator measurement approach will collect a mix of qualitative and quantitative data from all participating organizations with additional measures collected in the Innovation Learning Collaborative.
The measures* described in the tables in the PDF below will be used across the participating organizations. Additional details on the specific measure definitions will be provided by the launch of the program. The learning collaborative will provide assistance to support clinics participating in the learning collaborative in developing additional measures that are meaningful to them, if they choose to measure additional aspects of care beyond the requirements.
