What is the Social Needs Baseline Assessment Tool?
The Social Needs Baseline Assessment Tool provides insight into your organization’s capacity in the core domains of assessing for and responding to social needs. The assessment will be repeated at the end of the program, and the results used to assess shifts over time.
Results from the assessment will enable you and the MCU program team to understand organizational strengths, areas of opportunity, and progress over time.
Who should complete the tool?
The tool is meant to be self‐administered and completed by the core MCU team. We are looking for ONE response from each organization.
Some teams may choose to complete the tool individually, discuss the results as a group and then reach consensus on the responses that best represent organizational capacity. Others may complete the tool together. Depending on your team’s composition, it may be helpful to reach out to others at your organization for input especially senior clinical leaders.
How is the tool structured?
The tool is in the format of an Excel worksheet. The focus areas are divided in two ways. First, there are four descriptions of capacity in the focus area, ranging from no capacity/activity to a high degree of capacity/activity. The descriptions are further divided by stage, based on whether the activities are being tested at one or more sites, implemented in some sites, or implemented across all sites.
A score (ranging from 0‐9) is associated with each description and stage; these scores are used to calculate an average score for each domain.
How do we score ourselves?
To determine a score, first select the description that best applies. Then, consider whether the activities are in the testing phase, partially implemented at one or more sites, or fully implemented across sites.
Then select the corresponding number (0‐9) that best represents your organizational capacity. Enter this number into the column ‘Your Score’.
We realize that the descriptions may not be a perfect fit. Please approximate as best you can. If you believe your capacity truly falls between two descriptions, select the lower of the two.
How will our responses be used?
Your responses will help the MCU program team to better understand the MCU cohort and plan technical assistance offerings accordingly. Further, the data will be used to track changes over time. It can also help you in planning your activities and reflecting on your own results over time.
The data from your organization will remain confidential and used only by the MCU program and coach. The data may be aggregated in reports or publications. Any data shared beyond the project team will be aggregated and/or otherwise de‐identified. We will attain your advance permission before sharing or publishing identifiable data.
