About the Program:
In partnership with the California Health Care Foundation (CHCF) and Cedars-Sinai, the Center for Care Innovations (CCI) is launching the Connected Care Accelerator (CCA): Equity Collaborative — a new learning collaborative to help California community health centers to reduce disparities in access to telehealth for historically underinvested communities.
Organizations that are accepted into the program will receive grants of $75,000, access to monthly coaching, an online learning community, virtual convenings, and tools to reduce and/or eliminate disparities in access to telehealth for defined populations. The learning collaborative will run for 13 months, from April 2022 to May 2023.
The Request for Applications will be open from February 2, 2022 through March 16th, 2022 at 5pm PT.
