ABout

Join us for an engaging, actionable convening shaped around your needs. During the convening, you will collaborate with peers around shared challenges, elevate lessons learned, and co-design new solutions.

Breakfast and lunch will be provided. Attendees will be able to indicate dietary preferences in the registration form.

CONVENING GOALS & OBJECTIVES

⭐️ Goal

Improve the effective implementation of CalAIM through peer-led collaboration, resource exchange and ideation among CalAIM implementors.

⭐️ Objectives

  1. Share challenges, problem-solve together, and elevate promising practices to improve reach and services for:
    1. Enhanced Care Management Populations of Focus: Justice-Involved and Nursing facility residents ready to transition to community.
    2. Community Supports: Three support areas for Housing and Diversion to Assisted Living.
  2. Share and learn from peers about the use of the Incentive Payment Program (IPP).
  3. Build motivation and community amongst MCPs through peer learning and partnership building for CalAIM implementation.

WHO SHOULD ATTEND THIS EVENT 

Each team is invited to bring up to five team members. We encourage at least 3 team members to attend to engage in planning discussions and transfer lessons learned back to your organization. Please only register if you are confident you can attend.

Day 1 — Thursday, October 17 ( 10:30am – 4:30pm )

  • 10:30am – 11:00am | Check-in (tea and coffee available)
  • 11:00am – 1:15pm | Welcome, Warm-Up, and Content Sessions
  • 1:15pm – 2:15pm | Lunch
  • 2:15pm – 4:30pm | Content Sessions (con’t)

Day 2 — Friday, October 18 ( 9:00am – 1:00pm )

Preparing for the event

COVID-19 Policies/Procedures:

Please do not attend the event if you are feeling unwell. We will provide masks, COVID tests, and hand sanitizer during the event to use as needed. Color-coded accessories will be provided to signal social distancing preferences. Please refer to CDC Guidelines for additional information about COVID-19 protocols.
