ABOUT
Don’t miss our final gathering to celebrate your progress and accelerate your project’s momentum. Join us for an engaging 1.5 day in-person session, focused on MAT expansion, overdose prevention, and staff development and training. You will receive expert advice from coaches and network with your peers to build your plan for sustainability.
Who should attend this event
Each team is invited to bring up to four team members (representing your clinic and community partner). Grant funding is meant to offset costs of attending in-person convenings for your team members, including your community partner.
Registration guidelines: We recommend sending leaders (e.g. team lead) with decision-making abilities that can champion specific lessons learned. Please only register if you are confident you can attend.

PREPARING FOR THE EVENT
Pre-work
- Submit Photovoice by EOD May 3rd.
- We will have dedicated time in the learning session for teams to view each other’s photovoice projects and present their project to a peer team.
- The CCI team will print your photovoice project for you.
- Select Your Breakout Room: Complete this form by end of day May 8th to select which breakout room you would like to participate in. You will find descriptions of each breakout room in the form.
- Gift Exchange: In the opening ceremony of the learning session, we are holding a gift exchange. We ask that each participant bring something meaningful as a small, simple gift. It can be a baked good, a gently used item, something you knit, or whatever you want to gift to another participant in the cohort.
COVID-19 Policies/Procedures
We’re continuing to learn how to navigate this new phase of the pandemic with your safety in mind. One of our key goals is to make you, the event attendee, feel comfortable. To ensure that, we are asking all participants to adhere to the following safety protocols. We will continue to monitor all conditions related to COVID-19 between now and the start of the event and may revise health screening protocols at our discretion. We will inform you of any such changes so you can properly plan ahead. Please let us know if you have any questions.
- Masks: Masks will not be required unless they are mandated by local health ordinances. All participants are encouraged and welcome to continue wearing masks at their discretion. CCI will have available onsite both N-95 and surgical masks.
- Social Distancing: CCI will provide participants color-coded accessories (stickers, lanyards, bracelets, etc.) to signal social distancing preferences.
- Vaccines: CCI strongly encourages COVID-19 vaccinations for all participants attending in-person events. If you are not vaccinated for COVID-19 we ask that you wear a mask at the event.
