The Center for Care Innovations (CCI) hosted an informational webinar about the Roles Out Of Traditional Systems (ROOTS) Program. Supported by a generous grant from the Blue Shield of California Foundation, this program will support between 5-7 clinics over 12 months through a learning collaborative focused on the role of clinics in addressing the social determinants of health. The program will include online and in-person sessions, technical assistance, site visits, a SDOH related project that is a clinic and community priority, and grants in the amount of $35,000-$60,000. This webinar offers detailed information about the program offerings and application process. During the last section of the webinar, CCI hosts a question-and-answer session.
