May 1, 2020 Finalize your initial measurement plan. We will be asking you
to share yours by May 1st.
August 1, 2020 Develop a process map/workflow for your project.
August 31, 2020
  • Develop, refine and plan to identify and address transportation access or food security in your clinic population.
  • Start testing approach(es) in at least
    one clinical site
  • Measure, learn and modify!
