CCI has contracted with researchers from SIREN at UCSF to evaluate the ROOTS program.
The overarching goals of the evaluation are to better understand the roles CHCs can play in addressing patients’ social needs and the support and resources CHCs need to expand work in this area. The evaluation will include phone interviews with project leads and staff, surveys of staff, providers and patients, and a mid-year in person site visit.
