This is the final program evaluation report for the Expanding Access Through Team Care program.
In 2014 Blue Shield of California Foundation, in partnership with the Center for Care Innovations, launched the Expanding Access Through Team Care program. This program provided funding and technical assistance to health centers to improve their care teams and implement solutions that would allow those teams to see more patients. Through this final program evaluation report, the Center for Community Health and Evaluation looks at the the impact this program had on the thirteen participating health centers in their efforts to strengthen their care teams and improve access for their patients.
