As part of the ATSH:BH program, teams are required to submit progress reports, quarterly data and capability assessments, as well as participate in other evaluation activities.

Your organization will complete a detailed capability assessment which seeks information on MAT-related activities and infrastructure to support teams to better understand their current state, identify areas to make better, and evaluate change over time. This assessment will be repeated at the midpoint and end of the program. Download the excel template and submit via online form. Each organization participating in ATSH:BH is required to submit data on the required measures on a quarterly basis. Teams may select to submit data on additional optional measures. View the definitions for the core and optional measures by click the button below.
On a quarterly basis you will submit data on the required measures and any chosen optional measures via the data portal CCI has set up in partnership with the National Institute for Children’s Health Quality (NICHQ).
