PHASE Learning Community Convening: Toward Equity in Hypertension Care

Twice a year, Kaiser Permanente and the Center for Care Innovations bring together the 18 teams involved in the PHASE program for an in-person convening of the 2017-2019 PHASE learning community. The three broad objectives for our convenings: knowledge dissemination, skill building, and peer sharing.

Who Should Attend 

Each organization is expected to send three to five team members. Recommended roles include:

  • The day-to-day PHASE team lead
  • The PHASE clinical champion
  • Quality, Medical, and Nursing Directors
  • Data analysts
  • Front line staff working to improve blood pressure control among hypertensive and diabetic patients
  • PHASE champions who have been invited by their participating consortia

Entry is available from Jefferson Street or Martin Luther King Jr. Way.
