PHASE Learning Community Convening: Toward Equity in Hypertension Care

Twice a year, Kaiser Permanente and the Center for Care Innovations bring together the 18 teams involved in the PHASE program for an in-person convening of the 2017-2019 PHASE learning community. The three broad objectives for our convenings: knowledge dissemination, skill building, and peer sharing.

Who Should AttendEach organization is expected to send three to five team members. Recommended roles include:

On-site parking is available across the street from Preservation Park at the City Center West Garage for an hourly rate of $4. Entry is available from Jefferson Street or Martin Luther King Jr. Way. |
