As our health system continues to adapt to virtual care, our team has worked with members of the CCI community to compile resources to help you better engage your patients. You can also filter resources by different audiences, from the patients themselves to administrators.
It’s important to get buy-in from staff, outline roles and responsibilities to support patients using the devices, identify specific patient characteristics that might benefit from RPM, and identify the data needed to measure effectiveness. On-going care management processes and workflows are also key, so that readings from the devices can be integrated into the care process at the right time for the right provider. These requirements should be identified at a high level in the planning phase, however, once the specific characteristics of the devices and care management services are known, staff can be trained and processes can be defined at a more granular level.
