Catholic Charities of Santa Rosa

Coalition of Orange County Community Health Centers

The Coalition of Orange County Community Health Centers (Coalition OC) is the recognized leader for community health centers in Orange County. We work with our member health centers to promote the health of all people living in Orange County. Until now, this has meant treating illness and disease. However, with the growing anti-immigrant sentiment and fears of immigration enforcement, the Coalition OC cannot address patient health without first addressing the threats to our patients’ lives and families. Marin City Health & Wellness Center

African American youth drop out of high school at a rate significantly higher than any other ethnic group. The Marin City Academy of Health & Wellness High School is a pilot program combining preventative behavioral health support with a challenging intellectual environment, where teens experience learning, hope, career options and academic success. The Santa Cruz Community Health Center and the Live Oak School District are collaborating to improve health and educational outcomes through a unified Community Care Team that will leverage assets, overcome barriers, and institutionalize new systems and protocols to identify, treat, and monitor high-risk children. West County Health Centers

Guerneville, CA

When our patients do not have reliable transportation, we see an increase in no-show rates, lost clinic revenue, patient health risk, and inappropriate ER or hospital utilization. To help remove transportation barriers for our patients, WCHC will be partnering with Hitch Health, a proprietary technology that automatically initiates a patient ride offer through SMS texts using a ride-share service like Lyft when an appointment is scheduled.
