How to Apply

Step 1: WATCH THE Informational Webinar (Optional)

Watch the Informational Webinar to hear a detailed description of the program and listen to some questions and answers. The slide deck from this webinar can be found here.

Also, listen below to our most recent episode of CCI’s Health Pilots podcast, where we have one of our Connected Care Accelerator (CCA) alums, Dr. Mai-Khanh Bui-Duy from Salud Para La Gente, share about her experience working in the collaborative. Dr. Bui-Duy gives us insight into the benefits of engaging with her fellow participants in CCA, as well as how the program has supported her ongoing journey to provide telehealth.
