As part of the ATSH:PC, Wave 2, teams are required to submit progress reports, quarterly data and capability assessments, as well as participate in other evaluation activities. Learn more about the program measurement strategy by reviewing this website.

As part of pre-work, your organization will complete a detailed capability assessment which seeks information on MAT-related activities and infrastructure to support teams to better understand their current state, identify areas to make better, and evaluate change over time. This assessment will be repeated at the program mid-point and end. You may use this PDF document version if team members want to print out the assessment; however, you must submit the final assessment using the EXCEL template (downloads automatically).

On a quarterly basis you will submit data on the required measures and any chosen optional measures via the data portal CCI has set up in partnership with the National Institute for Children’s Health Quality (NICHQ).
