As part of the ATSH:PC teams are required to submit progress reports, quarterly data and capability assessments, as well as participate in other evaluation activities. As part of pre-work, your organization will complete a detailed capability assessment which seeks information on MAT-related activities and infrastructure to support teams to better understand their current state, identify areas to make better, and evaluate change over time. This assessment will be repeated at the program mid-point and end. You may use this PDF document version if team members want to print out the assessment; however, you must submit the final assessment using the EXCEL template (click to download).

Each organization participating in ATSH:PC is required to submit data on three required measures on a quarterly basis. Teams may select to submit data on additional optional measures. Download the document with measures and definitions below.

On a quarterly basis you will submit data on the required measures and any chosen optional measures via the data portal CCI has set up in partnership with the National Institute for Children’s Health Quality (NICHQ).
