TC3 – Share & Learn Session:

This is our first planned in a series of monthly Share & Learn webinars.

The COVID-19 pandemic has required that we all shift our work into the virtual space. As grantee teams are struggling to navigate the space, taking care of our TC3 populations is more challenging than ever. Through small group breakouts, teams can learn from one another to share what is working or not working – having access to “just in time” content to support them in testing / operationalizing new approaches and workflows.

The components of our monthly Share & Learn sessions will feature bright spots in a team’s current and evolving work in the field during this time, as well as have smaller group discussions for further sharing, insights, idea-generation and soliciting support.

In April 2020, our team at CCI interviewed Dr. Rachel Stern and Henry Rafferty with San Francisco Health Network. They shared some of their innovative work in telehealth and their EHR tools at the wake of this current crisis. In the initial portion of our May Share & Learn, we’ll have an opportunity to hear more from Rachel and Henry into their experience with outreach & chronic disease management during this time.

We will also to be joined by our expert faculty, Dr. Eric Henley from LifeLong Medical Care and Dr. Michael Mulligan from Chapa-De Indian Health, who will be co-facilitating the interview.
