Organization Description: Los Angeles County + University of Southern California (LAC+USC) Medical Center is the cornerstone of the safety-net for the largest county in the United States. As one of the largest public hospitals in the US, we care for nearly 60,000 Los Angeles County residents empaneled to our primary care medical homes. We are home to the largest internal medicine residency training program in the US, with over 165 resident providers.
Our mission is to provide fully integrated, accessible, affordable, and culturally sensitive care, one person at a time. Primary care services are provided in a safe, patient-centered environment through a network of 8 medical home clinics. The medical home serves as a hub for the larger network of hospitals, emergency departments, urgent care centers, behavioral health providers, and specialists within the Los Angeles County Department of Health Services.

MAT Services Description: LAC+USC Adult Primary Care MAT services are integrated into the primary care medical homes and delivered through a multidisciplinary team of providers that include nurses, psychiatrists, clinical social workers, medical case workers, community health workers, and substance abuse counselors. The MAT service model is delivered in a team-based model and referrals into MAT care can be made whenever needs are identified. Patients are connected to a primary care physician for MAT prescribing, a nurse care manager for complex care management, and integrated behavioral health staff for assessment, resource navigation and treatment. Most MAT inductions occur in the outpatient setting or are referred from post-hospitalization discharges. Integrated behavioral health services are delivered using the collaborative care model to maintain patients in a population-based registry for close follow-up and therapeutic interventions from the entire care team.
