We are excited to invite you to join a one-day event for healthcare providers in Los Angeles. The day will begin with a facilitated co-design session — a “design dash” — focused on integrating medical care and housing supports for community health center patients who are experiencing homelessness. The goal of the design dash is to identify current strategies and approaches and come up with ideas for improving care continuity and access.
In the afternoon, we will shift focus and hold several smaller breakout sessions. Attendees will have the opportunity to shape the conversation by suggesting any topic they think is important to discuss regarding care integration. People will vote for the topics they think are most important, and then we will hold three to five breakout sessions in the afternoon to discuss the most voted-for topics.
Breakfast and lunch will be provided. Attendees will be able to indicate dietary preferences upon registration.
WHO SHOULD ATTEND THIS EVENT
Chris has personally extended invitations to each of our guests to this convening. If you’ve already informed us about the number of team members from your organization who will be joining, we kindly request that all attendees complete their registration by October 4, 2024.
