WElcome to the Program!
We are so pleased to have you in Center for Care Innovation’s Advancing Behavioral Health Equity in Primary Care program. We are excited to launch this program together at our November 3 Kickoff Webinar. Before we launch, however, we would like to assess the strengths and opportunities of our cohort, particularly as it relates to Behavioral Health Integration and Equity. The Capabilities Assessment Tool (CAT) provides your team an opportunity to come together and evaluate your current state through conversation, and identify your focus areas for this Learning Collaborative.
Who should ATTEND this EVENT
Your Core Team participating in the Advancing Behavioral Health Equity in Primary Care Learning Collaborative and any other team members in your organization who can speak to the domains outlined in the CAT.
What you wilL LEarn
By the end of this meeting, participants will have:
- Understood the Capabilities Assessment Tool and its purpose for measuring progress throughout the course of the program
- Gained confidence in how to use and complete the Capabilities Assessment Tool
- Discovered ways the tool will serve the team’s collective ability to improve in key domains that contribute to successful Behavioral Health Integration and Equity.
