ReportingIn order to measure this initiative’s impact in expanding access to and use of specialty telehealth services among low-income Californians, participating health centers submit telehealth volume and usage data monthly.
Teams will also submit two narrative report on their progress in the development and expansion of their sustainable telehealth program.

This is the template for your monthly data reports.

Reports are due monthly on the second Friday of the month. Please submit your up-to-date information for all encounters (complete or in process) from the start of the grant through the last day of the previous month, all in a single tab on the template. You do not need to separate out the data in different tabs by month, etc.
