This webinar discusses the key lessons, impact, challenges, and recommendations from the past year of CCI’s CP3 Population Health Program.
In addition to CCI’s overall lessons learned, three representatives from clinics that participated in the program shared the changes they have made, challenges they have faced, and recommendations for other clinics looking to improve their population health activities in preparation for the APM. These participants were from San Mateo Medical Center, Tiburcio Vasquez Health Center and LifeLong Medical Care.
