ABOUT
Southcentral Foundation is an Alaska Native-owned, nonprofit health care organization serving nearly 65,000 Alaska Native and American Indian people living in Anchorage, Matanuska-Susitna Valley and 55 rural villages in the Anchorage Service Unit. SCF’s Nuka System of Care is a relationship-based, customer-owned approach to transforming health care, improving outcomes and reducing cost.
During this site visit, participants will learn more about their Nuka System of Care, explore their approach for overcoming barriers to behavioral health care, hear from an integrated care team panel, learn about their Family Wellness Warriors Initiative, and more!

Preparing for the Site Visit
COVID-19 Policies/Procedures
For CCI site visits we defer to the hosting organization’s COVID-19 guidelines. Southecentral Foundation’s COVID-19 guidelines are as follows: We ask that participants are fully vaccinated, however, Southcentral Foundation will not verify vaccination status at the site visit (an honor system). If you have not been vaccinated please wear a mask. If you have any questions please contact Isabella Chavez.
accommodation and transportation
CCI will cover the cost of the following items:
- Standard hotel room accommodations for 3 nights (the nights of May 17, 18, & 19 only)
- Networking dinner on May 18th
- Transportation from the Hotel to Southcentral Foundation (TBD)
Breakfast and lunch will be provided by Southcentral Foundation on May 18 and 19.
Note: Participants will be responsible for travel costs related to flying to and from Anchorage, Alaska, and can use RBN grant funds to cover the cost.
