CCI Team: Veenu, Juliane, Ray, Angela, Kathleen
Awards will be made based on soundness of project proposals and budgets. A successful proposal will…
Be thoughtfully designed with consideration to measures, sustainability, etc.
Have input from organizational leadership (CFO, COO, etc.)
Demonstrate relevance of the solution to other safety net organizations
Your seed funding application will consist of three documents. We’ve packaged up these three templates and made them downloadable here.
The pilot project charter outlines the who, what, and why of your project. The charter will help you communicate about your project and document the narrative when you are reflecting on your pilot experience.
The budget will help us understand how you are planning to spend the money and guide our funding decision. You may request up to $75K, though we may not fund the full amount budgeted depending on your plan. You may include budget for staff time.
As part of this metrics plan submission, you should consult with the SOLVE team. We’ve provided a template here, but you can create and submit any equivalent metrics plan that works for your organization.
SOLVE is a team of applied researchers based in San Francisco’s safety net healthcare system. SOLVE is working with CHIC teams to provide metrics and evaluation support for the CHIC technology pilots.
